- By Joining the “Smiles Club”, you agree to the following Terms.
- 1. Includes Free Routine Checkup and consultations (Upto Once in 3 months) including Intraoral Digital X-rays only as prescribed by the dentist for diagnostic purposes. Charges may apply for X-ray reports provided on Film/Prints/emails/etc.
- 2. Prior Appointments are must. Priority appointments given on a best effort basis but appointments for multiple members at same time are not guaranteed depending upon availability. Membership must be Declared at time of booking appointment.
- 3. Free Consultations may not apply to emergency conditions like pain/swelling/trauma/etc.
- 4. Applicable for In-House Doctors only.
- 5. Also Valid for Prosthodontics /Dental Implants/Crowns-Bridges/Dentures/Child Dentistry/Preventive Dentistry specialist Consultations. Not valid for Orthodontic/Oral Surgeon/Periodontist Specialist consultations.
- 6. Scaling and polishing includes only supragingival scaling.
- 7. Discounts Valid on regular prices. No other offers can be combined.
- 8. It is the responsibility of the member to Seek appointments to avail the benefits. Dentessence shall not be responsible for unused benefits, nor will the benefits be extended beyond validity dates.
- 9. Any changes or cancellations must be made at least 24 hrs in advance. Any free service would be deemed delivered and completed if the appointment is not rescheduled at least 24 hours prior or if the patient misses the appointment without information.
- 10. I may be contacted by Dentessence over Phone/SMS/Email/Whatsapp/etc.
- 11. Following regular maintenance schedule as prescribed by the dentist is important to avail warranty benefits.
